Prescription refills: Medication refills must be requested a week in advance. Prescription refill requests following a missed appointment will incur a $25 fee.
Requests for paperwork: Fees for paperwork requests vary depending on paperwork
Disability: We are not able to perform disability assessments or complete paperwork for disability claims Phone calls outside of appointment: Phone consults of more than 20 minutes will incur a $50 fee.
Payment Policy:
All co-pays and co-insurance are due at the time of service. We accept cash, Health Savings account, debit cards, and credit cards.
Please note: I am In Network with many Commercial Insurance companies (see list) and an Out of Network Provider. If I am not a provider in your network and you have out-of-network benefits, I can provide you with a superbill to submit to your insurance. Upon receiving this, they will send a check to your home typically within 7-10 business days. Payment is expected in full at the time of your appointment. By booking an appointment with Cross My Heart Wellness, LLC, you have knowingly, voluntarily, and specifically selected an out-of-network provider to provide services for you. By booking an appointment you are consenting and agree to pay for services rendered regardless of your coverage. You are responsible for finding out your coverage, managing your account, payment for & requesting a receipt for, and the balance of your bill for services.

Cancellations: We respectfully request that you give at least 24 hours of cancellation notice. This early notification will enable us to offer that appointment to another client in need. We are happy to assist in rescheduling your appointment to a more suitable time.
Appointments must be cancelled 24 hours in advance. Missed appointments without a call will incur a non-refundable $150 fee for the first appointment and $100 for follow-up visits that will be processed to the credit card or bank card on file. Patients who miss 2 appointments in a row will need to discuss an alternate plan.
